Dr. Siranush K. Cholakian, MD, a family physician with over 30 years of experience, specializes in integrating ancient and modern medical techniques to provide the most effective individualized treatment plans for her patients. The history of humanity and medicine are closely intertwined. In fact, the word “Doctor” comes from the Latin word “Docere,” which means “instruct, teach, point out.” As a patient, Dr. Cholakian will be your guide on your healing journey.
ABOUT THE DOCTOR
Dr. Siranush Kapriel Cholakian, MD has over 30 years of clinical experience. She is one of the select few who successfully obtained the Integrative Medicine Board certification when it was first introduced in 2014, and has subsequently been re-certified as of 2023.
Prior to that, she was certified by the American Holistic Medicine Board and the Family Medicine Board. She holds numerous certifications and diplomas for the diverse healing approaches she uses in her practice.
Her background in family medicine and alternative, integrative, holistic medicine enables her to heal a wide range of ailments for both young and old.
It is important to note that Dr. Cholakian believes in God, recognizing Him as the true healer who has provided us with herbs, remedies, and wisdom to improve our health and healing.

With over 30 years of clinical experience in her solo medical practice in Queens, NY, Dr. Cholakian has recently expanded to a second location in Stamford, CT, where she spends a significant portion of her time. Her Queens office is primarily office-based, while in Stamford, she enjoys providing home visits and improving the health of homebound chronically ill patients as well as other ambulatory patients at her office.
Her offices are designed to be warm and welcoming, providing a comfortable environment where patients can relax and focus on their health and well-being.
Telemedicine and Home Visits:
Dr. Cholakian also provide telemedicine services on a case-by-case basis for patients who prefer remote consultations or are unable to travel due to illness or disability.

Initial consultation with treatment plan (2 hrs): $500
Please have your medical records with you, to use for reference if needed.
Follow-up consultations (1/2 hr): $120

DIRECTIONS to the CENTER:
The center is located in the quiet scenic area of the Rego Park Crescents. It is easily accessible by car and public transportation. Street parking is readily available and there are no street cleaning restrictions.
By CAR from MANHATTAN:
LIE East to Exit 19 (Woodhaven Blvd and Queens Blvd, joint exit), Right onto Woodhaven Blvd., Left turn onto 64th Road.
By CAR from LONG ISLAND:
LIE West to Exit 19 ( Woodhaven Blvd and Queens Blvd, joint exit) go around curve , Right onto Woodhaven Blvd. Left turn on 64th Road.
By SUBWAY from MANHATTAN:
Take E or F, then transfer to R, M (local subways) to:
- Woodhaven Blvd. Station. Then transfer to the bus Q 11 for a couple of stops and get off at 64th Road cross the street – the Center is located behind the Mobile gas station.
- 63rd Drive Station(Subways R, M), transfer to Q 38 bus and get off at Woodhaven Blvd. Then walk to 64th Road. The Center is located behind the Mobile gas station.
From 63rd Drive Station (Subways R, M), you can WALK to the Center (10-15 min.). Walk down 63rd Drive towards Woodhaven Blvd. Turn left into Fitchett Street (the street just before you reach Woodhaven Blvd.). Walk down Fitchett Street for 1 block, you will see the Center at the corner of the second block on your right.
